Billing and Insurance
All payments, including co-payments, are due at the time of your visit, unless specific arrangements have been made in advance. We will ask you to verify your current address and insurance information at the time of each visit.
Insurance Referrals
If your insurance plan requires a referral form from your primary care physician, then you must provide the completed form to us by the time of your appointment. Otherwise, you must assume responsibility for the full charges incurred at your visit. If you are insured by Medicare then you are required to pay the annual deductible and/or the 20% balance of Medicare’s allowed charges, either personally or by a secondary insurance.
Outside Laboratory Services
We send blood tests to outside laboratories, which will then bill you for their services. The blood draw is available in our clinic. Please direct questions regarding those charges directly to the individual laboratory. We also have a state of the art endoscopy center on site, and billing for these services is performed similarly according to the guidelines above.
